Data use agreement

After requesting and reading the full Data Use Agreement, please take the following steps:

  1. Sign the Investigator signature block and other highlighted information.
  2. Identify your institution’s authorized representative to sign the agreement.
  3. Follow your institution’s procedure for submitting the agreement for the authorized representative’s signature.
  4. Note: When you sign the agreement, you will be able to download the pdf to provide to your authorized representative.
  5. Send the signed agreement to Jesse Philips at jap6ex@virginia.edu for UVA Authorized representative’s signature.
  6. Completed Agreements will be provided to participants.

Clarifications regarding the Requirements of Investigators (listed on page 2 of the Data Use Agreement) and the Obligations of Investigators, Research Staff, and Institution (listed on page 3):

  • Only one person per team needs to sign the Data Use Agreement.
  • If a team is composed of participants from multiple institutions, then at least one person from each institution needs to sign the Data Use Agreement and have it signed by an authorized institutional representative from their institution.
  • Graduate students can sign but there must be a person with a PhD on the team.
  • After one member of your team signs the Data Use Agreement via Docusign, you must submit it to your authorized institutional representative. After they have signed and you have sent the signed agreement to Jesse Philips (jap6ex@virginia.edu), we will send you a final form requesting your CV and information about the rest of your team (your "Research Staff" as defined in the Data Use Agreement).
  • In the event a graduate student signs the Data Use Agreement for your team, this is where you would list the team member(s) who have a PhD.
  • In the event your team has members from multiple institutions, this is where you would list the details about team members from multiple collaborating institutions.
  • All other obligations listed under Obligations of Investigators, Research Staff, and Institution (e.g., Data Security Plan, IRB approval, etc.) are covered by signing the Data Use Agreement and following the Data Security Plan in Attachment A (outlined on p. 11).

Data access via Globus

Secure data access is provided via Globus. When your Data Use Agreement is finalized, the members of your team will be invited to access the data via Globus and you will be notified via email. Please follow the instructions provided by Globus to create an account, verify your identity, and download the data.
